Philhealth Requirements for New Members






Life is hard and hospital costs are rising. In these times, one can
not afford to get sick.

Once sick, expect a lot of your money go down the drain.

Prepare for the worst, don't let poverty prevents you from insuring
your health. Be a Philhealth member.

It is very simple to be a new member. Just go to the nearest
Philhealth office and ask that you be made a member.

Requirements for new Philhealth members
1. Fill out the Philhealth membership registration form or (PMRF)
2. Attach the required documents
      Examples of required documents are the following:
      a. Birth Certificate
      b. Marriage Contract
3. Pay your contribution
4. You are now a Philhealth member, its that easy.

Your benefits are guaranteed as long as you pay your contributions
regularly.

The Following are just some of the benefits you as member gets
1. Are You hospitalized for Dengue, dont worry Philhealth will pay
   you a package rate of P8,000 for Dengue 1, P16,000 for Dengue II.
   The Dengue Package includes the following:
      a. Room and Board
      b. Medicines
      c. X-Ray and Laboratory
      d. Professional fees of Doctors
2. Are You hospitalized for Hypertension, don't worry, Philhealth will
   pay you a package rate of P9,000 for essential hypertension, the
   hypertension package includes
      a. Room and Board
      b. Medicines
      c. X-Ray and Laboratory
      d. Professional fees of Doctors
3. Are you hospitalized for Pneumonia, don't worry, philhealth will
   pay you a package rate of P15,000 for moderate risk pneumonia,
   P32,000 for high risk pneumonia. The Pneumonia package includes
      a. Room and Board
      b. Medicines
      c. X-Ray and Laboratory
      d. Professional fees of Doctors
